For a Group Application please fill out the form on our Home page and we will email one to you or you can contact any of our Board members directly and they will be happy to get you an application.

  All applications for 2013 membership are due by February 15, 2013. No group applications are accepted after this date. Please include contact information and a current group schedule of appearances with your application. We will begin accepting applications for 2013 group membership at our Annual Convention in November.
